Transportation Service is transporting a client to and from community resources identified during client assessment.

Who needs Transportation Service?

You may need Transportation if you:

  • Have no access to a working licensed vehicle or a valid driver’s license
  • Are unable to drive due to physical or cognitive limitation
  • Are unable to secure transportation from relatives, friends, or other organizations at no cost

Do I qualify for Transportation Services?

You may qualify if you are:

  • Low income
  • Medicaid eligible

You must first qualify for a DHHS program that has Transportation Services and a worker must determine that you need this service.

Where can I get transportation to take me?

You may get transportation for things like:

  • Going to medical appointments
  • Nutrition needs (for example, grocery shopping and participating in nutrition programs)
  • Your appointments or functions required by:
  • Department of Health and Human Services for benefit programs
  • Social Security Administration
  • Veteran's Administration
  • Financial institutions
  • Locating housing
  • Going to attorneys, Legal Aid Societies
  • Other needs identified during your assessment, like attending adult day services or receiving out-of-home respite services. These needs must relate to maintaining your independence.

Are there situations where I cannot use transportation service?

Programs do not pay for transportation for socializing, recreation, religious activities, or other trips which would be appropriate for volunteer transportation providers.

RTA Senior Rides Free Program--Registration Center

Medicaid Transportation Assistance For Disabled And Seniors

Medicaid clients are eligible for transportation assistance to non emergent medical appointments that are covered by their Medicaid Card. Medical transportation provides the client with the most cost effective transportation available. Medical mileage reimbursement, bus tokens, bus pass, taxi rides, special needs rides, non emergent ambulance rides, and rides with the Department of Human Services Volunteer programs are all options. A few clients who receive waivered services may be eligible for non medical transportation.

If you are eligible for S&DS medical transportation services, you will need to have these rides arranged through RideSource (special mobility services). You should call at least 3 days in advance to schedule a ride to a medical appointment. RideSource will verify eligibility, authorize, and arrange the transportation.

I am a Medicaid client. How do I schedule a ride for medical transportation?
Medical transportation is available for people who are eligible for full Medicaid coverage and have no other transportation options. RideSource will arrange for the most cost effective medical transportation with local providers. Medical transportation is provided to go to OMAP covered appointments. All medical transportation must be prior authorized.

Transportation options include gas mileage reimbursement, bus passes, bus tokens, taxi and ambulance services. A few clients may be eligible for non-medical transportation. Clients who are eligible for a bus pass can also use their bus pass for non medical activities. Medicaid clients should speak to their worker to find out which transportation option can be authorized for them.

Medical transportation is authorized from your home and back for medical services that are covered by your Medicaid card. Clients are encouraged to use mail order and delivery for pharmacy services. Generally medical transportation does not include trips to the pharmacy unless the type of medications cannot be delivered or mailed.

Some situations are not covered by medical transportation:

  • To a non-medical appointment—shopping, visiting, classes, etc.
  • To a medical facility that is not an OMAP provider or service (such as the VA hospital)
  • To pick up or drop off durable medical equipment or supplies
  • To an evaluation (including medical evaluation) for another agency or program that is not a service covered by Medicaid
  • Regular trips to a pharmacy.
  • Friends or family riding with you to your medical appointment are not covered.

If you will not be keeping a scheduled appointment or if you have a change (time, date, address, etc.) you should report that to RideSource immediately.

Which local private transportation providers are also Office of Medical Assistance Program providers?
This is a list of frequently used local private transportation providers who are also registered as Office of Medical Assistance Program (OMAP) providers. Remember ALL rides must be prior authorized through the Medical Transportation Desk (682-4038). If you need an unplanned, after hours or weekend, ride you must tell the provider that you are a Medicaid client before taking the ride. If you are an eligible Medicaid client the provider will bill Medicaid for the ride and you will not have to pay.

Any Warren or Center Line resident who is 55 years of age or older or handicapped individuals.

1. All trips must be scheduled through the dispatcher.

3. The bus/van will pick you up for shopping between the hours of 8:30 a.m. to 10 a.m.
4. We cannot specify an exact time. Please be patient.
5. The shopping limit is two shopping bags per person.
6. There is a $2.00 donation for all Warren & Center Line residents.

MEDICAL APPOINTMENTS:

1. Medical appointments must be scheduled 1 week in advance.
2. All appointments need to be concluded by 3:00 p.m. If you have to wait after you have seen the doctor, please be patient as we may be late picking you up.

Texas Medical Transportation Programs

The Medical Transportation Program (MTP) is part of the Texas Health and Human Services Commission. It helps CSHCN Services Program clients get to their doctor's office, drugstore, or anywhere that they get medical services. If you have no way to get to these places, MTP can help you. Go to their Getting a Free Ride to the Doctor, Dentist or Drug Store page.

Routine Medical Transportation

A van, bus, or cab service arranged by MTP to go to an appointment in your own or a neighboring county is called “routine medical transportation.” Please call MTP at 1-877-MED-TRIP (1-877-633-8747) to set up routine medical transportation. You must call MTP at least two business days before your appointment. This is a free call in Texas.

Special Medical Transportation

Transportation for nonroutine trips for medical reasons (including those where meals and lodging may be needed) is called “special medical transportation.”

Special medical transportationincludes:

  • Reimbursement for your mileage when you drive your own vehicle for services inside or outside of your immediate area
  • Prepaid bus tickets or airline tickets for trips of long distances
  • Travel by other means to receive services
  • The cost of a hotel when you must stay overnight for medical reasons

The CSHCN Services Program may be able to cover the cost of meals, a place to stay (lodging), and transportation for you and an adult attendant (such as a family member) if you must stay away from home overnight or longer for medical reasons. For all of these types of travel, MTP will help you arrange what you need for your trip. This includes meals and lodging in cases where the CSHCN Services Program covers those things. If you check with MTP ahead of time, in many cases you may be able to get funds before your trip to cover your expenses as you travel.

In many Texas towns and cities, MTP can make special arrangements for you to stay at places that are designed for children with special needs and their families, such as the Ronald McDonald House.

Oregon Ways To Work Offering Low Income Working Parents A Way To Receive Low Interest Auto Loans

There is an alternative for responsible, working parents to receive low-interest auto loans.

Ways to Work is a national program of Alliance for Children and Families that allows qualified working parents who are unable to get funds elsewhere, to receive loans up to $6,000 to purchase, repair or refinance a car.

The first program of its kind in Oregon, Ways to Work is operated locally by Metropolitan Family Service.

Loan Eligibility
To be eligible for loan consideration, a potential borrower must: 

  • Be employed three months continuously
  • Be an involved parent
  • Have exhausted other loan sources
  • Have a household income less than 80% of the Portland median income (80% for a family of 4 is $54,300)
  • Reside in Multnomah, Clackamas, or Washington County

The State of Illinois is pleased to offer People with Disabilities Free Rides.

This program offers people with disabilities who meet certain income limits, and are enrolled in the Illinois Circuit Breaker program, free rides statewide on all fixed-route public transit systems.

The Illinois Circuit Breaker program income limits are as follows: $27,610 for a household of 1, $36,635 for a household of 2, $45,657 for a household of 3. To apply for Illinois Circuit Breaker, go online to www.cbrx.il.gov or call 1-800-624-2459.

Once enrolled in Circuit Breaker, contact your local transit system (or any fixed-route system statewide on which you wish to ride).

PLEASE NOTE: The State of Illinois issues Circuit Breaker Program postcards to all currently enrolled persons with disabilities in the circuit breaker program. Circuit Breaker enrollment makes a rider eligible for free rides, and the postcard serves as proof of eligibility for the free ride benefit. Denver Access-A-Ride For People With Disabilities

What is access-a-Ride?
access-a-Ride provides transportation to passengers with disabilities who are unable to use RTD's regular lift-equipped fixed route bus service and who qualify for certification under the eligibility guidelines established by the Americans with Disabilities Act (ADA) of 1990.

Who is eligible for access-a-Ride service?
The access-a-Ride program is open to persons with disabilities under the following conditions:

  1. An individual requires a lift-equipped bus and the bus they need does not have a lift.
  2. An individual is unable to independently get to and from a bus stop or cannot get on and off the bus.
  3. An individual is unable to understand how to complete bus trips.

Architecture barriers not under the control of RTD and environmental barriers (e.g. distance, terrain, weather) do not alone form a basis for eligibility. However, the interaction of such barriers with an individual's specific impairment-related condition may form a basis for eligibility.

How do you qualify for access-a-Ride service?
The certification process requires a functional evaluation and a physician's statement for verification. There is no certification fee.

If you qualify for access-a-Ride, you will receive a photo ID card that must be shown to the driver each time you board. You will also receive a User's Guide explaining how the program works, including information on making and confirming reservations, types of service, escorts, trip cancellations and more.

Where can you go on access-a-Ride?
You can use access-a-Ride to travel within Arapahoe, Adams, Denver, Boulder, Broomfield, Douglas and Jefferson counties as long as the same trip could have been made on the regular RTD fixed-route bus system. Your trip must have a starting and ending point within 3/4 mile of an RTD non-commuter fixed route bus service as well as call-n-Ride service areas and you must travel on the same days and during the same hours as the fixed route bus service operates.

What types of services are available?

  • Reservation Service: access-a-Ride reservations can be made up to three (3) days in advance. Make your request as early as possible, since access-a-Ride trips are scheduled on a space-available, shared-ride basis.
  • Subscription Service: Recurring trips, same days and times, at least three times per week, you may request access-a-Ride subscription service. This allows you to take regular trips without making and confirming reservations each time. Please note that subscription service is very popular and there may be a waiting list.
  • Door-to-Door Service: access-a-Ride offers curb-to-curb service with driver assistance at the vehicle or curb. Special door-to-door assistance may be available upon request.

Family needs help because of transplants

Hi everyone her my name is michael my wife and i have 4 children . We started out doing very well but now we are having trouble with living expences. I had a kidney transpant 28 years ago and recently became disabled. My daughter also has become a transplant patient. She too had a kidney transpant and the donor ws her mom. I went from making 60 thousand dollars year to making disablity and it has been so roof. I have never asked for help in my life but i am now at the point i have to. I have asked churches to to help but as usually they only want to send their money and help to other countries , something i will never understand.  I am now in a wheeelchair and having trouble taking care ofmy kids so my wife had to stop working help at home.  Are car has broken down and we have been having trouble getting my daughter back in forth to Birmingham Alabama which is 5 hours away from where we live. If she does not get seen she takes chance of something going wrong and losing her new kidney.

I have learn one thing about get help with medication and that was how to aply for assitance with manufatures of the drugs and if there is anyone out here in that nee i can do all your paper work and help you to get the medication that you need all i would need is you to give me information or i can call the company get your application and send them to your with instruction on how to fill them out. If there is anyone out here that could help me and my family with expences or find a car please i would appreicate the help.

However, this method of accounting fails to consider the fact that poor workers without a car may not be able to search for or accept a better-paying job because public transit doesn't go there, causing these workers to lose lost income or benefits as a result. This report outlines opportunity costs experienced by transit-dependent poor households, and concludes that when all costs are considered along with benefits of private vehicles, it makes sense to press for more assistance and policies that reduce car ownership costs for poor workers."

The Car Ministry is dedicated to recycling cars and trucks to give to needy people and ministries. There is a great need for cars and trucks to ease the transportation burden. The program is dedicated to connecting those that are in need of a car or truck with those that can provide them for free.


Programs/Services Offered:
The 2C8 Car Ministry program accepts donated vehicles, checks their condition, and assesses the need for repairs. Then, the ministry gives the car or truck, at no cost, to someone in serious need, or the program uses the car to further the goal of providing transportation. The program has a long waiting list of people in need.


Results and/or Performance Measures:
To date, the Car Ministy has given over 2000 cars and trucks to needy families and ministries. They are assessed, reconditioned, and given away without charge.

The King County Domestic Violence Transportation program helps survivors of domestic violence travel to safety and to shelters. The program also provides transit tickets for victims who need to get to court hearings, support meetings, or medical appointments. Additionally, it provides a van to a community agency that transports victims to support groups.


Results and/or Performance Measures:
The program provides more than 400 annual emergency taxi trips to shelters, health clinics, and advocate agencies. It also provides over $35,000 annually in Human Services bus tickets. The program is a valued partnership between the King County DOT, the King County Community & Human Services Dept, the YWCA, and local domestic violence shelters and agencies.

The Residential Transportation Program identifies residents and staff in Seattle and King County Housing Authority HOPE VI sites and trains them to provide transportation information to residents.

Services included: 1) Multi-lingual travel training information to address needs of refugees and new immigrants; 2) Development of the Transportation Options Workshop and Residential Transportation Coordinator Toolkit; 3) Development of the Transportation Resources Index, a website of transportation services for individuals and communities.


Results and/or Performance Measures:
The RTC program was a partnership of agencies based on the block-watch model. The efforts of individual RTCs were monitored for information they provided, such as enrollments in programs such as vanshare, vanpool, ridematch, Working Wheels, or Accessible Service. The RTCs spoke several languages including Vietnamese, Arabic, Amharic, Oromo, Tigregna and Cambodian, making it possible to provide transit and rideshare information to these communities.

Traveling Grannies/Grandpas Program
Michigan

History:

The Traveling Grannies/Grandpas program was founded in Grand Rapids, Michigan. The program began seeking funding in 1992 and the first Traveling Grannie/Grandpas volunteer started serving teens in 1993. Since the inception of the program, 557 at-risk, pregnant, and parenting teens and their children have received in-home mentoring services.

Population Served:

Low-income older adults 60 years and over provide services to at-risk, pregnant and parenting teens and their infants. The Traveling Grannies and Grandpas were also involved with the Family Impact Project through the Grand Rapids/W.K. Kellogg Foundation Family For Kids Initiative. Through this project, 42 children ranging in age from 6 months to 15 year in the foster care/adoption system received hands-on mentoring/companionship.

Services Provided:

Low-income older adults volunteer 20 hours a week and earn a non-taxable stipend of $2.55/ hour along with other tangible benefits. The Traveling Grannies and Grandpas provide the following services which helps at-risk teens gain greater self-sufficiency and improve their well-being and that of their children:

Decrease the occurrence of abuse, neglect and exploitation of teens and their children by:
Teaching positive parenting techniques
Promoting non-violent disciplinary approaches
Building self-esteem by focusing on positive actions of teens
Deterring additional pregnancies
Promote positively life style choices
Discouraging the use of alcohol and drugs
Deterring gang involvement
Promoting continuing their education
Seeking gainful employment
Demonstration of good health habits:
Encouragement to secure pre- and post-natal care
Assisting with keeping medical appointments for self and child
Instructions on nutritious meal planning and preparation
Provide needed transportation:
To make applications for job openings
To attend job interviews
To keep all other necessary appointments.

The Traveling Grannies/Grandpas Program partners with several community organizations. The following list is a sampling of partnering agencies and is not meant to be all-inclusive: Catholic Social Services, Salvation Army Booth Services, Park School, Spectrum Health, St. Mary's Hospital, Family Independence Agency, Homeless Youth Services, Planned Parenthood, Kent County Health Department, One-Way House, Family Outreach, Arbor Circle/Project Focus, and Hope Community.

 

 

 

 

Job Opportunity Transportation Program
Ventura, California

The Human Services Agency (HSA) of the Ventura Department of Social Services operates the Job Opportunity Transportation Program, a self-funding program that provides people enrolled in California's welfare program (CalWORKS) with reliable transportation for work. The initiative's primary goal is to assist CalWORKs families in obtaining reliable transportation to maintain employment, get off welfare, and achieve long-term financial self-sufficiency.

The program operates as an auto loan initiative which purchases economy vehicles that fleet operators (such as local government or large private employers) plan to dispose of because of age or high mileage, or because the vehicles are surplus. The vehicle obtained are inspected, and all necessary repairs are completed by a private sector auto repair facility, or by auto repair classes at local community colleges. The vehicles are then sold at minimal cost to pre-screened welfare families to be used as transportation to and from work. The Ventura County Federal Credit Union provides loans to these families to purchase the vehicles.

Fix-it Ministry
Home Repair & Car Care -HIGHLAND MICHIGAN


The purpose of the Home Repair & Car Care Ministries is to serve the church family and the community. Everyone has home repairs and automotive needs that need to be done sooner or later. Not everyone has the skill, the time, or the tools. Calling a contractor or garage for the smaller repairs can be impractical and costly.

This ministry is intended to serve anyone who may need the help. From seniors, widows, and single moms to lawyers, teachers, and engineers - give us a call, we'd love to help.

A HELPING HAND MINISTRY (Car Give Away)

This ministry furnishes cars that have been donated by church members to church and community members. Persons who have old cars that are operational and in reasonably repairable condition should consider donating their car to the church. All donations are tax deductible.  Those interested in donating a car or who are in need of car should contact the church office.
